Help identify this 22re mystery clacking noise

New to me 1992 pickup, with 15k on a rebuilt 22re motor.
At idle the motor makes a strange clacking noise. Like someone's tapping on it with a hammer. It's not consistent either. It usually does it when it's warm but not always. I notice it most when I'm idling at a stop light. Again sometimes it makes no noise, sometimes it's pretty subtle, other times it's very loud like when this video was recorded.
Hard to tell where it's coming from.
adjusted the valves thinking that wouldn't hurt, new gasket, half moon seals and valve cover grommets. Torqued to spec (53lb/in IIRC). Metal chain guide.
Checked the idler bearing and it spins free and smooth.

Any ideas?

Rattle rattle PUFF, is what I hear that would be an exhaust leak or loose spark plug.

That PUFF you hear is the clacking. happens about once every revolution. Its not a puff and there are no loose spark plugs.

Kind of sounds like a high voltage pop. Like one of your spark plug wires are popping off to ground some where.

have you looked at it at night in the dark too see if the high tension leads are shorting?

any stumble when this happens?

I've posted about this before, YouTube video is not the right tool for audio, being able to see the wave form of the sounds gives a much better impression of what is happening when and at what interval.

"From here" what I hear is rpm related but not the same frequency as the revolution, again it seems to be on the exhaust stroke (half of engine speed).

You don't give a full tour of the engine bay, you should walk the camera all the way around all four sides. (It seems you are focused on the left side as the source..) Tell us where you think the sound is coming from, more so left right front back ect..

This would be a good use of the paper towel tube trick if it is not an internal sound (something you don't hear with a stethoscope pressed against the engine..

If it is not exhaust two other likely causes are a leak in the PAIR system (the small hose along the intake from air box), or the coolant fan rubbing either on the shroud or radiator.
