Basically, you'll need to beat the living hell out of the old one to get it off.

Unbolt the 2 caliper bolts... 17 millimeter if memory serves me. leave the brake line attached and have a bungee cord handy to support the caliper and keep the weight off the line.
To remove the rotor you will have to disassemble the hubs. Not a very difficult job, have a rag or tray clear and lay the parts out in order of removal. to reinstall reverse removal process. Also do one side at a time to have visual reference if you get stumped. Any questions pm me

I used two zip ties to secure my calipers to the upper a-arm, but it works just as easy. Assuming you only have ADD hubs, you just pry the cap off and remove the center hub bolt (12mm I think). You then pull off the hub (1 piece) and are faced with a huge spindle nut and spindle nut bearings. (54mm)... They look like this basically:

Then, the hubs pull straight off.... It isn't brain surgery by any means.... Make sure you tighten up the studs and everything real good.... May as well replace the pads while you are at it....
