rough idle at cold start-up

rough idle at cold start-up
'98 land cruiser with 208k miles that has been very well cared for - oil change every 5k, all fluids changed every 30k, big 90k service at 80k when purchased and then at 170k miles, new OME suspension...
so in the morning no matter if 60 degrees or 40 degrees (warm new orleans climate), a rough idle develops that will disappear in 10-15 minutes or so once the engine is warm. the rough idle is worse when vehicle is in gear vs. N or P. but in N or P the idle is higher at around 1100 rpm due to natural warm up for the first few minutes. the vehicle has no problem accelerating from the rough idle but at stop you can feel the rough idle and see the tach go up and down slightly. this problem was noticed at around 205k miles.
i took vehicle to my toyota dealer last week..new plugs, fuel injector/induction service, new fuel filter and the problem persists.
any thoughts? thank you!
