Cause of intermittent rough idle at start-up?

Any idea what could cause a very intermittent (couple of times a year maybe) rough idle at start-up? This has been going on for years and its not getting any worse so I'm not concerned about it - more curious than anything. It happens on both cold and warm starts and when it does, if I give it gas a few times it slowly gets better until its running normal, then it usually doesn't happen again for a long time. I replaced the fuel filter last year thinking this might help and it didn't.

Thanks!!!

My '94 does it also just for a couple of seconds and it returns to normal idle. Today it acually stopped doing that and it is back to normal.

Mine does it too, about every other time I start it up, when it is still warm, it will idle roughly for a few min until I drive around a little. Hopefully it isn't a HeadGasket, since I just replaced mine, and I have new injectors.

What makes mine different than yours though is that mine is very intermittent. It will happen maybe four times a year at the most - usually less. There is no rhythm to when it happens. It will happen once and then not again for many months, even though it is driven regularly. Also, I had my headgaskets done and it did it before and after this, so I think that can be ruled out.
