Fluctuating idle problem

Hey y'all, new to this forum! I have a 2001 tundra with 230,000 miles on that bad boy. I recently brought it with me on a move from Washington State to Germany, and since then it has been sounding/acting irregular. For the most part it WAS only at idle but now it's moved on to in drive as well. Basically, the tachometer spikes around 2-300 randomly, and as soon as it does, I get a low rumble/vibration coming from the front passenger side of the truck. I can feel it in the pedals, and it's been getting worse since I've had it and had to drive it to work. I'd appreciate any response! This has got me Very confused. Not sure how to post a video but I would if I did! It's definitely loud enough to hear!
