Is you check engine light working (i.e. on when key on and not running) and is it on when your driving?

does it smell like its running rich? possibility the cold start injector is sticking on dumping more fuel in and the truck cant burn it at idle. Not sure how much it compensates though.

It sounds like a Valve setting or Timing issue. If it is starting ok it sounds like there isn't a fuel delivery issue unless your fuel pump is intermitantly going out,or you have a semi clogged fuel filter,It sound like your valves are out of adjustment allowing the engine to run fine until it warms up. The timing could also be off. Hope this helps you narrow it down.

Might want to check out the EGR valve and modulator. When it is idling rough, tap on the EGR valve body and see if that smooths out the idle. The EGR valve may be sticking slightly open.
