engine code P0401

engine code P0401
hi my toyota 97 rav4 has enigine code p0401 and it is egr insufficient im trying to fix it but i just cant. i replace egr valve it self the vsv (vacuum switching valve) and the hoses. i also did fuel injection and cumbustion cleaner but it did not help me at all.... i also checked the egr vacuum modulator for operation and the filters are still ok i clean the throtle body and still no change. i dont know what else to do please help......
