possible problems with my A340H in 4x4 mode

Some things.
-Switching to 4lo ALWAYS hits hard, especially if you're idle is a little high.
-Not sure about the ADD, but with manual hubs, you're supposed to run the truck in 4 wheel for a little bit each month to keep the bearing surfaces lubed. It's very possible that something's dried up.
When was the last time you changed your front diff fluid? Transmission fluid? Greased the front driveshaft?
As for the shifter, get under the truck, and check all of the connections for the shift linkage. The A340H has a ton of them, and if they get loose, you'll notice slop.
