I have a 1994 4runner V6 manual with 110k miles. Havent done much to the truck except fluid changes. Also, it was worked on by Toyota for the Campaign for the head gasket defect at 60k miles

The smell in the exhaust is unbearable. In fact, I cant even lower my back window or open my window when the car is running. The smell is NOT a rotten egg smell. Just exhaust smell. It has been like this as long as I can remember but progressively getting worse. The smell is particularly bad especially when I first start the car when it is cold.

Is there anything I can look into to solve this problem? Never changed the O2 sensors yet. I am not very mechanically inclined, but know a thing or two.
