I have a 1991 Toyota truck with a 22re, the problem is that when its warmed up the engine goes crazy ie: tach jumps all over, it starts off normal then when Im driving it will start to hop and then the engine will follow, its almost like its not getting fuel. The engine stalls and dies some time when this happens I get the check engine light on its been throwing code 12, 13 14. I cleaned them out so that i can start fresh and find out what the heck is going on.

Sounds like a classic TPS problem.
there is probably other conditions, but i had a bad tps and the tach would jump around like that.
search tps, youll get tons of information

Ok I stumped and lost my mind and feel in my hands, its just too cold out.
the truck a 1991 toyota 4x4 with a 22re cali spec.
The problem:
The engine is stalling, hesitates, irregular engine speed, and dying. This only happens when it is at warm running temperature.
What I have done:
I have checked the TPS also clean it all out, distributor, coil, ignition module, wire loom, ecu, grounds. I fixed all grounds with sand paper and took them to bare sheet metal.
It is all good, I used an ohms meter and did everything to specs and read the most of the FSM on the systems.
EDIT: i have about done a full tune up today, adjusted the valves and spark plug gap.
I figured when stumped bounce ideas off peoples heads and something might come up.

start with checking the plugs around the igniter for corrosion or other issues such as the terminals are not fully captured by the plugs, water or condensation in the plugs, loose fittings, etc.

in the igniter the wires are hard and its the original, i did check that and I going to take it inside to get it warmer. that might free up any water.
the primary coil 1.5 ohms (took reading form the + and -) the secondary was 9.3 ohms (took from + and high tension) this was take at cold.
