Perhaps a valve adjustment but first I recommend checking the fuel delivery system. Start by finding and replacing the fuel filter. Then go on to check the O rings on the injectors. I had a similar problem on a 22re and one of the O rings was busted and if it sat for any length of time the fuel system would lose pressure and it'd be a bear to start, not as bad as yours but still a pain.
That thing sounds like it isn't getting fuel at start up and is starved for fuel when you hit the throttle by hand.
Oh and post up a bigger picture of your avatar. I want to get a better look at the...... um....... "fenders" on that rig. *grin*
