Question Engine suddenly rough with choppy feel

The story continues.
In the past 2 weeks I replaced head gasket and fuel injector gaskets.
22re would not stay running. I pulled the dist and and put back in tdc and then was running great.
Went to gym with 4runner and when I got in to leave I noticed that it felt like under powered with a hard idle that you can actually feel while driving slow or fast.
Other that a small exhaust manifold leak due to a stripped bolt I dont know what changed from the gym to after.
Any ideas? one of injectors get reclogged, timing off again, the exhaust manifold leak?
About ready to shoot Trigger and get a new horse.
