Just a few other places to start

Bad O2 sensor or wiring
AFM spring loose or broke
Short in wire from injectors to ECU.
Is the fuel return line free flowing.

Extreme slim chance that distributor is over pulsing the ECU and should throw a code.

If the cold start injector is stuck open or the switch that powers the csi.

How did this start? One member of this board recently had this problem. He had removed and re-installed the vacuum hoses, and in the process connected the fuel return line to a "vacuum" line instead, so he was pumping fuel directly into the plenum.

You could have a stuck-open injector. The CSI is easy to test; just pull it out of the plenum and start the fuel pump with the jumper on the diag connector. The other 6 require more work. Here's a picture of how to do it:

A failed regulator (or the vacuum plumbing to it) can over-pressurize the rail, which will lead to running rich. To catch that you need to check the fuel pressure.
