Is it my head gasket or something else?

I have a 1987 22RE that was rebuilt about 18 mounths ago by a local shop. There is no water in the oil. Nice clean oil. 160 lbs of compression on all 4 clyenders. No bubbles in the radiator when warm and ideling with the cap off. It runs strong down the freeway at 70 mph.

But it just started smoking like a fiend, radiator water is going away and water is almost running out the exhaust. The exhaust water smells like coolant.

Do a leak down test. It will tell you if it's your head gasket or not.

If you're burning that much coolant, it's the head gasket. Sorry.

i had the same problem on a 22r carbed and a 22re. theres a small plate on the back of the head it has i around 4 bolts holding it on. it was the gasket on that was gone on both of mine. seemed like the coolent was basically pouring out the exhaust. once i fixed those gaskets no more smoke or coolent loss on either motors.

I pulled the head, no obvious leaking. The gasket had a defect at the cylinder edge beside a water passage. The bolts were not all that tight, but they were not lose. The water was only going out the #4 cylinder. Some signs of combustion gases leaking between 1 & 2 and 3 & 4. The head was not even stuck to the block. Good machine work where the block was decked and the head was straight.

I think the shop used a poor quality gasket to save a little money.

I always use OEM toyota head gaskets. There $47.00 and in the long run, it will save you money over that cheap junk.
