"My engine is smoking please help"

I need help people, this is my 2nd yota but this one i really am the one that has to fix it. Anyway i have a 89 4runner 22r-e engine, and i have had problems with it smoking some since i got it. But the engine sounds wierd, when i hit passing gear it makes a loud noise in the engine and then smokes and takes off and smokes the whole time. I can barely pull hills with it but the take off is great. I have a video i would like you to watch, also there is a screw looking thing in the video that connects to the ?? power steering vaccum?? Can someone tell me what it is? This yota has 189k miles. Also i think i am leaking oil from the front seal. Will i need a puller to fix this? Here is the link to the video's..

also there is a pic of a spark plug to let u see that too.. Also my back window doesn't roll down and my door doesn't open with the key, I have to take the back panel off and manually pull the cable to get the door ajar, but since the windows doesn't roll down it won't open.. I think i got into a mess here..

kinda sounds like a diesel. It also sounds like the valves may be clicking. It didn't happen to ingest any water did it.

ok, but wonder why the SMOKE? Is that oil smoke?? can u see it in the video? Also when the truck warms it doesn't sound bad, it sounds like a toyota that is revvd too high, i can't get the idle down much at all, i turn the screw and it goes so far but if i run the ac, she dies on me or almost close to dying on me. Oh well once i get the engine right then i can go from there..

As mentioned.. Adjust valves. Do a compression test.
If all the bolts were loose on the pan, someone has been in there..
