This is on a 1990 4Runner (different steering set-up), but outer TREs are all pretty similar- for instance, a Jag XJS I worked on was pretty similar to this as well.

You'll want to find a "pitman arm puller" for the job:

99 4Runner does indeed have 4 tie rod ends, but the inner tie rods are attached to the rack and rather dificult to replace.

Might be a better idea to find a low mileage unit from a wrecking yard.

i just did the inner 'rack end' (as toyota calls them) and it took me about 2 hours start to finish. i could probably do the other side in about 45 mins now that i know what to do. definitely need an alignment after all 4 are done, so have that appt. lined up.

you have to take the outer tie rod end off to get to the inner, so the job is pretty straight forward after you get to that point. i elected to swap out the rack's bushings while i was under there, too. a little extra work, for MUCH improved performance.

steps:
jack front end up.
remove wheels.
pitman arm puller on outer tie rod end. remove OTRE.
remove boot (cut inner clamp).
remove inner rack end from steering rack.
re-assemble.

new parts:
tie rod ends x4
boot clamps x2
cotter pins
pb blaster
loctite
bushing kit, again, have to suggest it!

price is a big consideration, too: ~$140/ea inner rack end, ~$40/ea outer tie rod end. bushings ~$30. alignment was $70 at sears.

i had to replace the inner end because it failed at the annual inspection. otherwise, i thought it was fine (i thought, initially, they were failing my truck so they could do the $$$ work on it to get it into spec). when i took out the end, it seemed like it was a little loose.

between replacing the driver's side inner end and the bushings, the truck felt REALLY tight after its first test drive. it was probably a combination of both factors that led to the huge improvement.
