Strange Grinding noise (bad pilot bearing?)

Strange Grinding noise (bad pilot bearing?)
OK, so the past few months I've had this strange sound coming from somewhere, and I can't quite determine if it's the throwout bearing, or if it's something else.
The sound happens most of the time now, but it only happens when the clutch is pushed in. Thus, the reason I think it's the bearing. But, sometimes I'll push the clutch in, and then hold it in, and as soon as I put it in a gear, it starts making the noise. Then it will slowly start to go away. Then as I start moving and I push the clutch in while in gear, it will make the noise... pull it out of gear with clutch in, noise stops, put back in gear keep clutch in, noise starts, as the truck slows down with it in gear and the clutch in, the noise will slowly stop, Then, after awhile, it won't do it anymore until it's colder. It's like something is lacking some lube.
Also, when I'm sitting at a light in first with the clutch in, and I hear the noise, if I let out the clutch just a little bit to jet the clutch plate touching the pressure plate, withou tthe truck moving, the noise will stop, then sometimes (50/50) after I push the clitch in, it will start again. Sometimes it makes no more noise.
I've recently changed the transfer case oil and the manual tranny oil thinking that one of them was dry or something, and I had never done it. So, that's not it. I just want to know if I can fix this, or if I should just be considering a complete clutch replace if I'm going to replace the bearing.
Any ideas?

That makes the most sense to me... The one thing that confused me most was that when it's making the noise with the clutch in, reving the engine doesn't make and change in the sound... maybe it's just rubbing up against the pressure plate, and beginning to damage it.
