P0136 hell

Rig:

'02 Limited
TRD headers
Volant CAI
URD simulator
brand new DENSO front and rear O2's

Problem:

P0136 routinely comes on (at least once every two weeks).

Backstory:

I can't seem to get rid of this damn code, whenever I'm putting around I seem to get this code to pop up. If I drive like a jackass it never comes on. I've replaced the TRD header gaskets with ones that don't suck and verified they're not leaking.

I'm not sure what else can cause this other than the URD simulator being broken (only ~3 months old).

Any ideas or diagnostic things to check?

I have no experience with that particular code but I would recommend doing an extensive search on here as well Google to see what ideas are currently out there.

Also, what exactly is this code?
Have you narrowed it down to a particular system/component?

GOOGLE search found this.... http://www.obd-codes.com/p0136 Not familiar with the URD simulators but I thought they were used to fool the ECM by reading the O2 parameters within specs yet you've got brand new O2 sensors? Either sounds like you have bad O2 sensor, URD simulator, or a connection problem.

I ran a URD Rear O2 Sensor simulator on my 3rd gen with no issues for 25k+ miles.

Might check your wiring on it to make sure it is still all intact...

Now that I read the link from Ironmike (thank you btw) I do remember this code and it is similar to the one I threw right after installing the URD simulator.
Turns out URD sent the wrong wiring diagram with the simulator so I had it wired incorrectly, once I got a copy of the correct diagram it worked flawlessly.

So, my initial recommendation of checking the wiring appears to still be a good one.
