Bet It's Your Input Bearing
The noise you hear is probably your front bearing on the input shaft of your 5-sp. transmission. Same thing happened to my 88 truck. Fourth is 1:1 so you don't get any noise. Fifth is probably the noisiest. Most likely you will eventually need a new transmission. I put in a heavy-duty Marlin transmission and clutch recently. It's great. No more f----ing noise!
Give Marlin a call. You'll be happy you did.

I noticed you have a Downey header on your 88. I just got an 88 myself. How do you like the header and did you have any issues getting it to fit? I noticed some kind of bar going across the top of my header. Can that be removed to use an aftermarket header?
