99 4runner hard to crank especially when cold
We have a 99 4runner (2wd w/ 4cyl engine, 210K) and have two problems which I do not know if they are related. - This vehicle has suddenly become very unreliable about starting up, particularly when cold. Turns over fine; then sputters and puffs white smoke over and over, but will FINALLY catch up and then run FINE. After this initial start up and while still warm it cranks ok; most of the time.
We have cleaned MAF sensor and removed the throttle body and cleaned it,also replaced fuel filter (which was dirty). It almost seems like it has jumped time, but if so then it would'nt run like a top after finally cranking?? Also, it tends to sporadically run hot. No water in oil and vice versa. I am at wits end, any and all suggestions will be greatly appreciated.

When was the last time you gave it a good tune up??
Changed the Air filter??
How cold is cold ??
It is normal to smoke some on cold starts .
Head Gasket if your blowing white smoke then your coolant level will drop has it??
I don`t know enough about these engines .
Do they get air pockets??
Have you done a pressure test on the cooling system??
Has the timing belt ever been changed

When it runs hot it sometimes blows out coolent through the cap on the overflow tank. The top hose will sometimes collapse. The 4cyl has a timing chain, so I don't think thats it. As for the start up: when it cranks it does so in 2-3 sec.s of engine turnover; when it won't, we usually have to hook up jumpers for enough juice to keep it turning while manipulating the acc. pedal till it finally stops sputtering/popping and catches up.
