Well my trusty 4Runner turned over the 200,000 km mark on the way in to work tonight. Wouldn't you know it would happen when I don't have my digi-cam (damaged cable).
So far this 4Runner has been trouble free. (fingers crossed).

Its jinx time! I tured over 200K and at 201K guess what: water pump. 202K water pump, timing belt, cam seals, misc timing pulleys, belts, tune up @ $996. 203K and now a major noise in the upper engine area like a cam or a pulley sound. still @ 203K and brake booster leaks air, warped rotors, and who knows whats next. Have a nice day!

Congrats on the mileage! I'll probably hit the mark in December or January.
It's a piece of machinery...as much as you baby it things will wear out and break and not always at a convenient time. It might be time to keep a little breakdown fund. You also might get underneath and check out everything you can, u-joints, idler arm, swap out your gear boxes lube and look for suspicious filings. But mainly listen...with a stethyscope or just a piece of garden hose. I even buy things on Ebay before they have gone bad. Clean them up, rebuild them and stick them on the shelf. They're ready for when I need them.
You can keep it runninng a LOOOOONG time if you just stay on top of things that are about to break down.

I recently hit 200K in Salina, Kansas on a trip home from Colorado. Right inside the border of Oklahoma my alternator died, other than that no problems.
