hub problems

I have a 1994 toyota pick-up running 37x13.5x15 tires on 15x14 wheels with 538 gears. My problem is it keeps blowing front drivers side hub about every 3000 miles. The tread on the front tires wore extremely fast the last 2000 miles. When I take it apart everything is tight and has plenty of grease.Anyone know what the problem might be?

The wheel hub or the manual hub? Please elaborate. How can it be tight and have plenty of grease and be "blowing"?
