You might as well, if for no other reason than to give you peace of mind down the road.

I would hope that toyota would install a new belt as part of the job. But you're right that you can't make any such assumption as far as a dealer goes. So yeah, check with them about it. And don't let them charge any extra labor for it, since it must come off as part of the head gasket job.

Water pump, depends on if the current one is an Aisin (which is what the dealer sells) and how many miles on it. I replaced an Aisin that had over 160k miles on it and it turns out it didn't need replacing - no leaks, bushings still tight and smooth like buttah, no wear or erosion on the vanes whatsoever. If it's not an Aisin, have them change it, since most of them don't last near as long as Aisins. If you do have them change the pump, it will cost extra labor since it doesn't need to come off to change the head gaskets.

The question is how many miles on your timing idler pulleys? The upper idler in particular is kind of a pain to change when just changing the belt, but it will take ZERO additional labor to change it when pulling the heads since it has to come off as part of that job. If the current upper idler has over 50k miles on it, have them put a new one of those in, but don't let them charge a penny extra labor for it. For the lower idler, same thing as far as no extra labor, but I would change that one unless it only has 25k miles or less. In my experience, the upper idler lasts longer than the lower. Upper, if using stock, should last two changes of the belt (140k miles). Lower, at least the ones I've bought from the dealer lately, show signs of worn bearings at only 70k miles, so should be changed every time.

EDIT: They will almost certainly replace the cam seals, but I would also have them replace the front main seal (crankshaft seal). That will cost extra labor, too, but they shouldn't charge more than .5 hours since the job can be done in 5-10 minutes, assuming the timing gear comes off okay.
