The 4Runners with the tire carrier have a built in support behind the outer body panel.

The swing out spare tire carrier has a spot welded in mount around the tail light.
I bought one off a local guy that had a 4Runner for parts. I asked him to simply cut out the rear corner for the bracket.
A good sawzall will do the trick.
Then you can cut out the spot welds holding it in and then either weld it in yourself or bolt it on.
I'm planning on bolting mine on to the back of my pickup. Bolt the latch to the tailgate and it should be plenty strong.

The tail gate has a latch that you will need, it goes into the recess where the license plate goes. You only need the latch piece.
