Loose flywheel bolts binding clutch?

Recently I my truck started showing all the symptoms of a failed Master or Slave cylinder. The issue started immediately, and was not drivable. It was as if the clutch was permanently engaged. Unfortunately, entire hydraulic system had been replaced within the last few months, and I was seeing plenty if travel at the throw out fork. After bleeding and bleeding, just to be sure, I knew the problem had to be internal. So I pulled the trans and clutch assembly. I found that several of my flywheel bolts had vibrated loose. Almost completely, and were worn down from rubbing on something.

My question is, has this ever happened to anyone else? Should I retighten the bolts with locktight and see if that fixes it? I'd hate to drop the money on a new clutch ($400-500 from UK/AUS) if mine is fine. I'd also hate to reassemble everything, only to have to take it ALL apart again and buy the clutch anyway if that's not my problem.

The truck is a 91 Xcab, with a 1KZ-t diesel, using the original R150f transmission. I've had no clutch issues in the past year or so since I got it running finally.

I'd chase the threads in the crankshaft with a tap, degrease the holes with starting fluid or similar solvent and install ALL new factory correct bolts. Hopefully, theres no damage to the crank threads. Locktite and tightening to proper torque values goes without saying.

You ought to be able to tell by visual examination whether the clutch is fit for further service, but if it was me, I'd at least change out the friction disc.
I can't imagine that loose flywheel bolts could damage the pressure plate if they did'nt come completely out.

I'd carefully inspect the crank end and flywheel mating surfaces too.

Did you notice any noises or vibration before failure??
