It accelerates great from a dead stop, as soon as I climb past 25 mph, it is guttless and seems to be starving for fuel. Choked out. I press the pedal to the floor, but it slows down. After a few minutes it gets its power back. Like I said this only happens occasionally when I drive. It is on and off. I am sure the tranny has issues as well and is close to needing a rebuild, but I want to know what the problem is with the engine. Sometimes I put the pedal to the metal at 30 MPH and it downshifts from 3rd gear 2500 rpms to 2nd gear 3000rpms and remains the same speed, trying to speed up but cannot.

Also when i start it up, the engine makes this rattling noise against the firewall which sounds like an exhaust leak coming out of a hose underneath the intake manifold. And when the engine warms up, the noise goes away. It takes 5 minutes for the engine to warm up as well. The idle sometimes drops to 100 rpms and dies when it is cold outside and I have to give it alot of gas to get it started.

COuld the on and off loss of power be the Idle aIr Control valve? MAF sensor?
I cleaned the throttle body a week ago and today cleaned it again because it was grimy already because I am also getting oil in my k&N air filter. I have a K&N intake and the tube gets burning hot also after I drive it. Could the MAF be causing the flap to stay shut at times? Fuel Pressure regulator?

I believe the questions being asked is: with the transmission engaged and the vehicle is moving under its own power. It is not just sitting at an idle with the transmission in park. There is no load placed on the motor. It's running and does not have anything that would put a load on the motor causing it to have to work.

Sounds like it might be the fuel pump. Have you tried jumpering it and checked with a gauge to see what the fuel pressure at?

park it on the street and then:
step on the brake pedal,
shift the trans into 1
step even harder on the brake pedal, and floor the gas pedal for 10 seconds,
and step as hard on the brakes as you have to so the truck doesn't move.

What RPM does the engine rev up to?

MY 4runner revs to 2100 RPMS WHEN I do this test!

it is supposed to rev at 3ooo because my dads revs at 3000rpms.
What does this test show if it revs lower than usual like at 2000rpms.

I hooked up the OBD1 sensor a few months ago this summer and it had a code 71 for EGR programmed in its history. As soon as I reset it and have been driving it ever since, the check engine light hasn't came back on, and no more codes are showing up.

I use to have the same problem w the rpms went up n down after that i replace the knock sensor an thats over
