What's slipping

Hello all.
I need help with a problem. Here's what's happening. When I downshift and acceleration something starts to slip. The majority of the time the slippage is after a downshift from 5th to 4th to accelerate up a hill. Sometimes it slips when I accelerate without down shifting. It doesn't slip every time and if I ease off the accelerator it quits slipping. It started slipping during the Winter and seemed to quit this Summer. Don't know if that's a coincidence or not. I checked the transmission fluid and it was fine. What should I do next?
Thanks

My 1986 4-Runner is a 22RE, 4X4, 5 speed MT, with 296K.

Definatley a worn or oil soaked clutch disc.

Without a doubt. Once they start slipping...it gets worse. Fix it know before you waste a flywheel.

Thanks everyone. I plan to replace the clutch this winter. The last couple of times I drove it the clutch hasn't slipped. Any thoughts?
Thanks
