Rough idle and stalling when starting engine?

Rough idle and stalling when starting engine?
My 2002 V6 is getting close to 130K miles and I am seeing alot of little issues pop up.
The newest is really rough idle and stalling when I start the truck after its been running a bit. RPMs run up and down between 1500 then down to stall out point.
No CEL indicator , and on second start up try I rev the throttle and it will stop sputtering and get with the program and run down the road fine.
I have a freeflow filter but its been in there for a few months with no issues.
Any ideas?

I did 1/2 can of seafoam in the PCV tube, then dumped the rest plus one more full can in the gas tank that had less than 1/2 tank in it.
Puked white smoke for 10 minutes, then I drove around a bit.
Trucks runs better than ever now.
