Unhappy Water mixed with oil and water ontop of Piston

Ok I have recently been having trouble with my 1983 22r. The entire engine has been rebuilt top to bottom maybe 4 or 5 month ago. When i got the engine rebuilt it ran with no problems at all!! They bored it out due to wear but am not sure what the measurements are. Now I have ran into this problem with this milky white mix internally all over my engine. I figured this was a blown head gasket causing water to mix with oil so I drained the oil pan and the first sight was water gushed out and all the milky residue. Then when I was getting ready to adjust to top dead center I removed the 4th spark plug and started cranking manually to TDC and water shot out of the cylinder so that wasn't a great sight Removed the entire head and there was water on the 4th piston closet to the cab and was rusted! What could have gone wrong ?! All along it has been running on 3 cylinders! The gasket doesn't look bad what could be the problem/s??

It has been overheated twice

Warped head, cracked head. Cracked block. Who assembled the engine? Was the head checked? Overheated since the rebuild? If so, then this should have told you something was wrong. I would take it back to the mechanic that put it together for you.

Well i took it to some mechanic that I was told was great and he did a complete over haul but when i took apart the head I looked under where one of the water jackets is and there seems to be a missing piece and It never overheated till maybe 1 month ago it started losing fluids quick and i was always having to refill it!

Pictures would help. Don't know what a missing piece could be.

Well theres your problem. Your head is invisible. Musta been pretty hard to work on and get it right.

Its not invisable, you don't see the red X?
