96 Tacoma overheating again

My 96 tocoma still overheating. I've replaced my head gasket, water pump, thermostat, radiator cap and timing belt. I've also bleed the air from my cooling system. I pressurized my cooling system and it held at 15-16 PSI. Found no leaks. I need help desperately. I don't know what else to do.

Under what conditions is it overheating? Standing still? Driving at low speed? Driving on the highway? Did you replace the head gasket to fix this problem or had the gasket failed? How old is the radiator/how many miles are on it?

It started to overheat when I went up a steep hill. I had my head gasket replace because it was blown. But the radiator is original. 146,467 miles.
