stagger or miss in fifth 3vze

First off, a big thanks to all of the posters here on YT. Finally solved my tick, tick, tick issue by reading posts about the steering column intermediate shaft seal! Saved me from pulling the injectors. Huge difference with the new gasket.

I have been experiencing a seemingly random miss or stagger occasionally when I'm in fifth gear. The truck lurches momentarily and then rolls right along. Doesn't seem like I'm lugging the engine and it doesn't happen all of the time. No check engine light. Starts right up and runs great. 91 4x4 3vze V6, top end rebuilt at 120,000 when HG blew, has 152,000 on it now. The truck did this before that. The truck is timed and tuned. Have not replaced the 02 sensor. Do they cause problems before failing completely? Blew a lot of water through my cat converter when the headgasket went. Doesn't seem to be restricting the exhaust. Any easy way to check this? Any other suggestions?
