Word has that neat feature in it to search for templates on line, but I am not finding want I want yet.

What I am looking for is a template with a Christmas theme background or a graphic at the top of it.

Then below an area that says:
Name Item bringing

This way on the sheet an office worker writes down their name and what they are bringing.

Anyone have one made up already or know where to get a nice one instead of making it from scratch?

We are having a potluck on the 14th.
