What causes Code 25 (Lean)?

What causes Code 25 (Lean)?

Just to hit up all my options of what could be my problem. I have ruled out the O2 Sensor, so what else could be causeing my engine to display an engine lean code while throwing out rich fumes?

How did you rule out the O2 sensor?

Check vacuum lines for leaks to start. Is the engine idling high at all?
Use a can of carb cleaner and spray on one hose at a time while engine is running.

The engine is idling very rough and low. I don't think it is a vacuum leak as I had a mechanic do a smoke test and all was well. I'm am rather confused about this problem.
