Help me Diagnose

Hey guys I've had my 4Runner for alil over a month now and I love it. It drives great with the exception of a couple issues. The problem is when I'm coming to a dead stop it feels like the transmission is hesitating gearing down to first. I'll be stopped and I'll let my foot off the brakes a little bit and you can feel the vehicle sorta jolt but it does not make a noise just feels like it shifted down. It just seems like it should already have shifted before I'm completely stopped. Also and I duno if its related is I get a loud clunk and jolt when I'm stopped and I put it into drive. Does anyone know what my problem could be?
