Driveshaft backing out???

Driveshaft backing out???

I will try and get a picture of this later today guys but at the moment i am in class. I was looking under my yota the other day and noticed something. On the driveshaft where it connects to the transmission (transfer case?) there is a gap between the case and what appears to be a cover that is supposed to go flush and over a bit of the case. the gap is about an inch and a half. Serious problem or do i just need to push this cap back up even with the case?

another thing i noticed was that there are only 2 original bolts in the tranny mount.. the third hole is empty and the forth has some long bolt with a lot of washers on it there... so that is a need to be replaced...
