Sounds to me like you failed to properly torque the wheel when you put it on.
The wheel is shot IMO.

Yep, time for a new wheel, and maybe even new wheels. Either you didnt tighten them enough, or you used the wrong lug nuts.

Live and learn.

x3. Don't kill yourself over a wheel.

I thought all new aluminum rims required a periodic re-torquing so the sleeves can get tamped down...

Vary common problem. If aluminum...

That and "knock off" spindles reversed so torquing spins them off, instead of tightening ask me how I know... No don't!

Your balancer guy will tell you if it's warped or not...

If it comes up to like 10oz. that's a TON in wheel weights, and it's warped...

And they make clip-ons for aluminum too, and you can set a balancer to only put them on the inside.

And yeah, x2 on 2door4runners post... "i can still centre the rim with the tapered lug nuts" Makes me think your using your original lugs...

See SR5 lugs. They're made to go into the lug hole.

You should just replace it as it is not worth an accident. That happened to me a few years ago when a buddy was helping me put the wheels back on my Altima. I assumed that he had tightened and centered the wheel properly. The wheel was toast and I could not find the discontinued style anymore. That really sucked but lesson learned. Double check your buddys work.
