Does anyone have a list of materials need to do this swap? Can i just go to the salvage yard and pick up what i need?

Rear discs swaps are pretty common. There are quite a few threads on this topic and several vendor kits available if you will do some simple searching for "rear disc swap"

 

This what I have:

98 Taco front discs
98 Taco front rotors
98 Taco Harware and hoses

#1 Do I need to remove the axles to install the rotors?
#2 Is this all I need beside the brackets?
#3 Can some one please give me a step by instructions
