Worn clutch?

Driving yesterday I could hear what sounded like the clutch skipping. It almost sounds like the engine is bogging down but the sound comes from the tranny area/rear of car and happens when the engine is defiently not bogging down. The sound is a constant "thut" "thut" "thut" at certain RPMs, it alsmost sounds like that annoying sound you get when you open a rear window in other cars and air piles in the back of the car. I also have noticed that the clutch pedal is grabbing late in the clutch throw. Is it time for a new clutch...? Any other ideas what it could be?
