Whats the best way to get more HP 22R-E?

Sounds like John has you on the right track!
Here's .02 on my '86 4Runner 22-RE.
Gears: I run 4.88's w/35ish"(315/75R16's) Baja Claws daily on interstates(speedlimit is 55, I cruise at 70) and stop lights. Granted I'm also at sea level here in VA, but at 70ish in 5th I can pass without downshifting. If I make it back to CO, I'm probably screwed. LOL!
Pipes: I'm using Downey ceramic coated header and they're exhaust system. Which includes Magnaflow hiflow muffler and cat. The pipes are 2.5" and I haven't notice any problems.
Motor: Stock. Cooling system: 3 core radiator and flex-a-lite black magic fan. The fan...has been the thorn in my side. With the bugs worked out, it works great and I do notice an improvement in MPG and HP. K&N FIPK kit which I'm debating on keeping. I also hand made one of those "tornado" things out of a hydraulic fluid can and some duct tape. It works.
Bottom line, as a DAILY DRIVER I'm getting 18 MPG. I do go and flex(as it were) around and play in the mud occasionally. I definately don't "grandma" around either. I flog the crap out of my truck.
My truck "feels" like it did when I had 4.10's and 225/75-R15's. A buddy of mine HAD(idiot) a '92 pickup 22-RE with 35x12.50's w/5.29's(I think, may have been 5.13's) and I didn't like it.

Guys like me can help a bit, maybe 10-20 ponies tucked in there but with big tires you lug the engine down and she might no longer be a fat pig but she will still be a pig.
Think gears! There is nothing I or anyone else can do to best that one change for oversize tires. Even at that, you lost, the mass of the bigger tires and wheels plus loading exceed the recovery in gear ratio, you are better off with smaller tires and wheels.
Yea, I know. Show up at the rock run with 10" wheels and yer 20" knobbies and it's giggle time, I don't care how high yer ladders are!

The next best step is a cam. THINK here!! (Ask questions!) Do you want to cruise on the highway and not need to get out and push when a hill comes along? Or do you want power all the way through the powerband, the heck with best peak power?
Maybe you have a lighter rig, a 2WD pickup or a Celica? Things your supplier needs to know to make an educated guess.

No matter what you do, no matter what anyone says, you will end up with around 135 to 140 ponies to use. More than that and we need to really talk, everything becomes a can of worms and a PIA.
I did build a high altitude engine some years back, she made 161 H/P. Spooky and I said it will blow up and I am right, it will...
Someday....*LOL** I also built a 6 to one over blown Celica, I told him it would blow up too, and I am right. It will. But he drives it to work and can't race anymore, seems 10.91 1/4 mile times require a roll cage.
Some of that can be addressed, please bring your checkbook. Serious past 140 ponies is *spensive.
