Warm 22re runs super rough. No problem when hot or cold.

Hey folks, I have a brand new intermittent problem that started yesterday. Here's what I think I know:

Cold start - OK! (sitting for a couple hours after last drive)
Hot start - OK! (sitting for 10-15 minutes after last drive)
Warm start - BAD! (sitting for 30-90 minutes)

If I park the hot truck for about 30-90 minutes, then try to start it...it misses horribly, puffs black smoke that smells like gasoline from the tailpipe, and will only stay running with the accelerator floored. If I can keep it going for a minute or so, the problem disappears and it runs normally. Something must be making my lukewarm 22re think it is much colder than it actually is, and resulting in a super rich condition. I feel like the shop will have a hard time diagnosing the problem because its somewhat intermittent. Any thoughts?

Update: Pulled the codes and it's only throwing #71 (EGR valve), which has been on for a couple years despite cleaning the valve repeatedly. I don't think it's related, but it's strange that it can run so rich without throwing a new code...

update: replaced the coolant temp sensor and sender, which seems to have fixed the problem for now...
