First grease them up really good everywhere, then you install them like the parts are show in post #3 pic. Just keep in mind that bolt is broken.

Head of the bolt down, nut on top, the half moon shapes go against the adjuster part attached to the t-bar. The part on the left fits inside the center part. The parts on the right stay together.

I grease up all those parts, even under the head of the bolt because there is so much metal on metal contact with a heavy load (aka lots of friction).
