My guess is FEL-PRO Part # 60876 . RockAuto has them for $1.13.
Of course, I'll bet you'll have an easier time finding one if you knew what make-model-year-engine you had. I can only guess it's a Toyota pickup 22re, and I have no idea what year.

You could always use gasket material. Just put the material over the tb and press on all the holes and they will leave indentions
Then simply cut them out with a razor blade and some scissors.
A huge roll is cheap and makes ALOT of gaskets.
