Spark Plug Problems

A couple weeks ago I thought I fried a piston ring in my 94 v6 pickup. But after a friend checked it he told me a spark plug had shot out of the block. I fixed it but was wondering how that could happen.

Someone didn't properly tighten it when installing / damaged threads.

ive seen em shoot out when not screwed back in properly. makes a lot of noise, sounds like the exhaust fell off. Probably worth checking the threads as stated above to make sure they're not torn up.
