Is my timing belt slipping? (Pictures)

I was wondering how accurate I have to set cam timing. Crankshaft is at TDC.
Here are the some pictures.
This is the right side


Now the left:


I would say the left one is pretty on point but the right on is half a tooth off and there is no way to adjust that because it's half a tooth.
Does this mean the belt is losing its tension?
Is this way my car is running so bad: not starting correctly, not revving up easy at low RPM(but after past 2krpm, it revs nice) I cannot image half a tooth of will make the truck undrivable.

HELP!

One thing I notice is your passenger side sprocket is on backwards.

Drivers side is flange out towards radiator
Passenger side is flange in towards motor.

Really? Nothing has been touched in regards to the sprocket..
What about timing itself? Is it safe to say it's good to go?

Check your tensioner adjustment. I've had to fiddle with mine in the past after changing the timing belt in order to get everything to line up correctly. reset the tensioner adjustment then spin the engine 2 full revolutions and see if everything will line up.

I've never worked on that engine before but I can see an arrow on each sprocket to indicated TDC so I will take a guess and say that they are both on the correct way. If its like my 7M, the sprockets will be identical and can be swapped to either side and will only fit on one way making it impossible to mess it up.
