Coolant leak help

I tried searching but didn't find anything similar. I'm not a total noob but it's definitely possible that I messed up a search.

I have a leak at both ends of the hose shown in the picture below. Has anyone seen this?

Here's the background:
I was sitting still in traffic and noticed no heat from vents and the temp was moving up on the gauge. When I started moving the heat came back and the gauge dropped. I made it to work but had to keep the revs up to keep the temp right and the heat coming. Later in the day (once the engine cooled) I ended up adding about 1/2 gallon of coolant to the radiator to fill it.

I changed the thermostat thinking it might be sticking and not letting coolant through, but the leak remained.

The leak is a slow drip at each end of the hose. All hoses, water pump, and t-belt were changed about 20k miles ago.

Any help would be appreciated. Thanks.

Half a gallon? Yikes... be careful or you'll blow the head gasket!

I'd drain some from the bottom of the radiator and make sure the coolant is ok. When it gets too low it tends to cook itself and get dark and/or rusty colored. Next, tighten the heck out of those hose clamps! You don't want leaks, ever. If they still leak, replace the hose but be sure to scrape off the crusty fluid first so it has a smoother surface.

After that, keep checking for leaks. Your reservoir (just to the right of your photo) should always be half full of coolant: if it's low or empty you've got a leak. If it's filling up (rare) you've got too much pressure or the coolant is boiling.
