Haha i fixed it. I was digging around in the engine bay on my lunch and found a ground wire just hanging there under the intake manifold by the fuel rail.

I hooked it back up and it has been fine for the past 3 days. I will try and get some pictures up and figure out where the heck it actually came from.

This is a very informative post. I have the same issue on my 3.0 that I just bought. I gave it the full tune up and the light came on. i know that there are exhaust leaks at the crossover so this might be the culprit. I also just failed my smog in WA. I failed three out of four tests for too high a reading. Might try to clean out the EGR replace the O2 and get the exhaust fixed and hopefully should fix my problem. Let you guys know what I get, but for now off to Dutch Harbor.

add checking the fuel dampener to the list sometimes the adjustment screw wiggles out causing a severe lean condition when accelerating. all you have to do is take off the cap with a magnet in your hand so you do not loose the screw in the engine like I did and see if it is loose or out completely if so replace the dampener with a quality denso dampener. my symptoms were lean on acceleration so much so that I had a lot of hesitation when I accelerate and was a gutless wonder.

also add replacing the injector seals since that is one thing that is commonly overlooked when looking for intake leaks also don't be afraid to cover the intake hoses silencers and resonators with duct tape to rule out those leaks. also I would test the ignition system last since lean condition are usually caused by too much air and or too little fuel.
