Code 12 and 25 diagnostic NEED HELP ASAP!!!!!!!!

Ok I have a problem, recently went through an emissions test, failed the first time, put a bottle of Guarenteed to pass in the tank, failed the second time, here are the readings

First test
idle
HC Limit 200 Reading 126 Pass
CO Limit 1 Reading 2.39 FAIL
NO N/A

40km/h
HC Limit 83 Reading 150 FAIL
CO Limit .46 Reading .43 PASS
NO Limit 893 Reading 1386 FAIL

Second test

Seems to be worse after the guarenteed to pass was added, I also followed the instructions for it to a T.

Now I checked the engine codes today and I have 12 and 25. I ran a can of seafoam through the intake and let it burn off, then changed my plugs, inspected the wires and removed and cleaned the egr, I tested the egr by removing the vaccum line while the engine was running and applied vaccum to the egr, engine began stuttering, indicating the egr is now working fine, The vaccum pressure seemed low and I will be checking that tomorrow night. So I will let you all know.

I also checked my o2 sensor for the code 25 problem(lean mix) and found that it is only ouputing apprx .13 volts, which to me indicates a dead o2 sensor. Is this correct? keep in mind i checked at the o2 sensor and the diagnostic port. Also checked the ecu for air fuel mix, got less than 1v which indicates the ecu trying to run lean due to a failure of either the o2 sensor or a saturated EVAP canister. Which do u think it would be and how do I check to see if my EVAP canister is saturated?

Also the cat is old and I plan on getting a replacement ASAP.

The code 12 I have been told could be a faulty knock sensor or a problem with the RPM signal, I get a clean readout on my tach, so I am wondering if anyone knows what it could be and could try and explain to me the possible causes/solutions.

Will doing the o2 and cat fix my truck and allow me to pass the etest? I know that no one could know for sure but do u think just doing those 2 would effect the etest that much so I can pass it? do I need to get the code 25 fixed as well? Also I bought a replacement cat and niether end has a stamp indicating the flow. How do I figure out which way to put the cat?

I realize it is alot and TRUST ME I have been lookin in the forums for answers for the last 10 hours. PLEASE ANY HELP WOULD BE SOOOO AWESOME!!
