Auto Trans Problem

The automatic transmission on my 95 SR5 is really making me crazy. I'm not an auto trans guy, this is the first one I've had in decades, now I remember why.
The transmission works pretty well. If I use the shift lever, it runs through the gears fine, driving normally, it shifts fine. However, every once in a while, when I accelerate and the trans drops into "passing gear", it makes a sick rattling noise, like if it was in too high a gear for the engine RPM. If I stomp on the gas and it goes to the next higher gear, the noise goes away. It only makes that noise in that one situation.
I've changed the fluid and adjusted the throttle cable, no help.
Like I said, I don't know crap about these Yota transmissions, ANY advice or insight would be greatly appreciated. I've searched around but still need more help.
