95 automatic 4runner surges when flooring.

This started happening today on the highway when accelerating hard. This is with the throttle wide open. I've tried it in neutral and it works fine but under load it surges like this. I don't know if it's a transmission problem or something else. My other guess was the fuel pump as it was really bad at a 1/4 tank and seemed to be less of a problem after topping it up, I've suspected the fuel pump of going recently because of it's poor performance when low on gas. But I'm not sure, is this characteristic of a dying fuel pump? Any other ideas? Help is appreciated.
