'93 4Runner throwing code 25

I've checked nearly every possible cause of this code 25 (running lean) and cannot seem to make any progress in solving this problem. I've checked for vacuum leaks with carb cleaner and inspected every hose on the truck. they're all in great shape.
The car now shifts rough and idles high, this all started as soon as the check engine light first came on. I've read that this can happen with the code 25. I've been chasing this thing for over a month now and would love to get this behind me.
Any and all suggestions are are greatly appreciated, thanks guys!

When you post that you have "checked nearly every possible cause" of something, what kind of answer do you expect? Do you want us to guess which "possible causes" you haven't checked? Not easy, particularly when you don't tell us what you think "every possible cause" is.

The FSM tells you what to check:
• Engine ground bolt loose
• Open in E1 circuit
• Open in injector circuit
• Fuel line pressure (Injector blockage, etc. )
• Open or short in heated oxygen sensor circuit
• Heated oxygen sensor
• Ignition system
• Engine coolant temp. sensor
• Volume air flow meter (Air intake)
(vacuum leak is not in there, but it IS worth checking on a truck more than 5 years old. Which is all of us.)
